Clergy and Congregational Care

Brief Description of Program:
Our Clergy and Congregational Care program operates as a unique service within Catholic Community Services.  The service provides Religious Congregations and Dioceses with a comprehensive and holistic response to their changing health care and support needs as their members age. Presently our service is being used by four Diocese and six Congregations. 

 

Our aim is to assist and empower individuals to maintain their health and wellbeing, to support clients in accessing appropriate services, to meet their changing health needs as they age and to ensure a continuum of care.

Our comprehensive assessment and ongoing review of care needs enables our support services to be appropriately tailored to the individual. Our approach to care supports and encourages the “Ageing in Place” model by assisting people to live independently with the support of our staff.

 

This program can assist with: 

  • Advocacy
  • Case Management
  • Counselling
  • Social Support
  • Transportation
  • Pre and post hospital admission support
  • Coordinate services for domestic and personal assistance
  • Report on trends and emerging issues to Religious Congregations and Diocese

Eligibility Criteria: An arrangement with Diocese and Congregational Leaders
